* run: FAILURE:: Only 1 instance(s) of mysqld running. 2 instance(s) required!
One or more module in class "proc" FAILED

Solution

EPAP having the alarm was Standby Server and Running in Release 16.1.1.

We found that we have had Syscheck error for mysqld process not running :

Running modules in class proc...
* run: FAILURE:: MINOR::5000000000000002 -- Server Application Process Error
* run: FAILURE:: Only 1 instance(s) of mysqld running. 2 instance(s) required!
One or more module in class "proc" FAILED

We checked the cgi.dbg logs and found getting DB_ERROR :

We tried to restart EPAP Services from CLI, but unable to start the EPAP services.

[root@mps-a logs]# service Epap stop
~~ /etc/init.d/Epap stop ~~
EPAP application stop Successful.
[root@mps-3001-a logs]# service Epap start
~~ /etc/init.d/Epap start ~~
setParam() failed with "DB_ERROR"
/usr/TKLC/epap/bin/uiEdit failed with return code 3. Is the UI database running?
[root@mps-a logs]#

We have had to reboot the EPAP A using "init 6" which bring back the server with all process running..
