Goal

SBR Blade not coming up after reboot, getting below error during server boot:

dev/hda6: UNEXPECTED INCONSISTENCY; RUN fsck MANUALLY

Solution

We had a problem before where COMAGENT Conn was in FORMING State for this SBR Blade and Getting error “ERROR-Lost communication with iLO” observed on OA for this SBR blade.

To fix the issue of iLO, customer engaged HP team and as per them Server has to be powered off and then has to do the below action :

  • Reset the iLO and NVRAM by manually using the management switch.
  • Once the iLO issue resolved and they went to do the power on, we see that the server boot is getting stuck at below place:
    During the boot, server showing below error
    dev/hda6: UNEXPECTED INCONSISTENCY; RUN fsck MANUALLY
  • During investigation, we Found BIOS Time is wrong which is showing 2 Nov 2014, which is incorrect.
  • The BIOS time was set to a 2014 date , which I am assuming is the time of when the BIOS image was produced.
  • Upon boot, the lv complained that the last time for the superblock was in the future (2017) and to run fsck
  • Rebooted and set BIOS time to current date and time
  • Rebooted and the server was able to be recovered with no alarms

Issue resolved setting the time from BIOS.
