Solution

Apply Exadata Software image 12.1.2.3.2 or later

Apply patch: Patch 23234193: CELLSRV FAILS WITH ORA-7445 [_ZN11DISKIOSCHED20PICKPRIORITYWORKLOADEV] on top of 12.1.2.3.0

Workaround

According to Bug 23234193: CELLSRV FAILS WITH ORA-7445 [_ZN11DISKIOSCHED20PICKPRIORITYWORKLOADEV]

You can change the IORM plan from "allocation" based plan to "Share" based plan.
