The ODA rescue disk procedure is the same as other Linux operating systems. Using the ILOM steps to attach an OL Linux ISO downloaded from https://edelivery.oracle.com/ 
For example, you can using OEL6.7 iso to boot into rescue mode to repair, investigate or change settings that may result in boot looping, incorrect hugepage settings, CRS problems or other boot issues.

Goal

 How to attach a rescue disk to the ODA

Solution

1) Steps to download an Oracle Linux (OL) iso from edelivery.oracle.com

Go to edelivery.oracle.com and choose Linux version or use steps from

         Where to Download Oracle Linux and Oracle VM RPM Packages and ISOs(Doc ID 1607100.1)

2) Connect to the ODA ILOM; use the ILOM redirect and then attach OL iso as a CD

Use steps as provided in several notes including

        ODA Oracle Database Appliance Bare Metal Restore Procedure X5-2 (Doc ID 1373599.1)

Once the ISO is attached using the ILOM redirect: follow commands to manually boot up in rescue mode

 

3) Linux boot-up steps in rescue mode

Summary: 
1) Started an INFRA Patching session

2) After a reboot, the ODA could not get out of a rebooting loop....

 ODA: INFRA upgrade to 12.1.2.6.0 failed during reboot: VFS Unable To Open Root Device "VolGroupSys/LogVolRoot" (Doc ID 2145985.1)


Resolution Steps:
1) (Doc ID 2145985.1) asked to download OL6U7 DVD Download from edelivery.oracle.com , however we tried that and it failed to boot.

     instead we have used Patch 24356630 EXADATA diagnostics.iso instead

2) User has problem in mounting and booting up from virtual cdrom .

We get over that by :

SSH to ILOM, then run
set /HOST boot_device=cdrom
reset /SYS
start /SP/console
root password is : xxxxxxx

3) after that we continue to follow step 5 an 6 of the (Doc ID 2145985.1) to fixed the corrupted rpms.

4) In order to start upgrading node 2 , customer need to clean up all the un-needed files in /boot to free up space.

user may remove files
under /boot
with *kdump* files
*245* <--- old kernels.
