During a switch firmware upgrade multiple MPEs did a QP Proc Restart.  This document provides some background information.

Solution

Below is the RCA for this issue

ISSUE:
While doing firmware upgrade on swtiches, MRA and MPE blades had qp_proc mgr restart.

ROOTCAUSE:
Firmware upgrade causes the Ethernet ports to restart. This issue happened because eth ports toggled

WORKAROUND:
In a procedure where servers could go into split brain like a firmware upgrade, it would be best if the non-active servers are set to forced standby so that they do not go active while the firmware upgrade is in progress.

FIX:
Documentation to have steps to put non-active blade in FS
