Sun Fire X4540 Server - Version All Versions to All Versions [Release All Releases]
Information in this document applies to any platform.

Goal

Sun Fire X4540, find the causer for what reason fault LED on HDD may still switched on, even after defective hard disk has been already replaced.

Solution

System administrator may check whether service LED is probably illuminated for affected hard disk because SVM mirror is out of sync or resync is still running.

Further possible issue causer could be a unresolved/open Solaris FMA event. For reference please have a look on;

Oracle Solaris Administration: Common Tasks (Repairing Faults or Defects)

http://docs.oracle.com/cd/E23824_01/html/821-1451/glird.html

First step, check current FMA status by using command "fmadm faulty -a".

For example;

fmadm faulty -a

                  not present

Description : SMART health-monitoring firmware reported that a disk
              failure is imminent.
              Refer to http://sun.com/msg/DISK-8000-0X for more information.

Response    : None.

Impact : It is likely that the continued operation of
              this disk will result in data loss.

Action : Schedule a repair procedure to replace the affected disk.
              Use fmdump -v -u <EVENT_ID> to identify the disk.

For resolving open FMA event use "fmadm acquit fmri | label" command.

For example;

fmadm acquit dev:///:devid=id1,sd@n5000c500149f8781//pci@0,0/pci10de,377@a/pci1000,1000@0/sd@0,0

Then reset SP and check status LED again for affected hard disk.
