SolutionNote: This is a disruptive test that should only be done during system idle or maintenance time when no active IO is occuring.
1 - Make note of the fc hba port LED pattern of the working fc hba port.
2 - Make note of the fc hba port LED pattern of the NON-working fc hba port.
3 - Swap the fc hba end of cable between the non-working and working fc hba ports.
If the NON-working fc hba port now shows the working LED pattern then issue is outside the fc hba port. Check components between fc hba port and fc switch/storage, as well as, the fc switch/storage port.
If the NON-working fc hba port still shows the NON-working LED pattern then fc hba card will need to be replaced.
