Action Plan :

$ boot -- -r

Why : This will eliminate any false entries.

If still issue persists, Then go to

B)

1. For now just rename /dev/cfg ( we will delete later when we confirm the issue is resolved)
$ mv /dev/cfg /dev/cfg.orig

2. Stop all applications and do a reconfiguration reboot
$ boot -- -r

When the system comes up, it will not find /dev/cfg and will create a new one.

3. Check that the newly created controller numbers/disks are consistent.

4.Once confirmed, delete the saved directory:
$ rm -rf /dev/cfg.orig
