VSM/SVA Clean Code Request Form

WARNING: A Clean on a VSM or SVA product deletes all pointers to the data.
After a Clean has been initiated, all data is lost and is NOT recoverable.

The person requesting the clean code is responsible for its use.  Be aware that all customer data will be lost.  If there are questions, open a support case to VSM Hardware Tech Support requesting assistance. If necessary, reference any previous case for historical information.
If the box to be cleaned is in response to a failure situation, then provide the current case number, or open a new case to VSM Hardware Tech Support for assistance to verify that CLEAN is the appropriate action to be taken and that it is safe to clean the box.

To request CLEAN microcode:
1) Open a support case for VSM Hardware Support and paste questions and answers from below into case.
Note: VSM and SVA products are supported by VSM Hardware support

2) Answer all questions below and paste the information in the case.

a) What is the Oracle email address of local FSE?
Clean code will only be mailed to a valid Oracle email.

b) What is the product and version that needs to be cleaned?

c) What is the functional microcode?

d) What is the Serial Number?

e) What is the Site name?

f) What is FSE manager’s name?

g) Why does it need to be cleaned?

3) After the box has been cleaned, delete the CLEAN SRL from the subsystem.
