X6270M3 (X3-2B) blades exhibiting FMA faults SPX86-8003-PD on both CPUs.

On power up the blade will see FMA fault fault.io.intel.iio.misc-vpp-fail on either or both CPUs at the same time.

Workaround: Clear the faults in ILOM using following procedure.
1. First start a fault management shell:
-> start /SP/faultmgmt/shell
2. Then clear the fault on both CPUs:
faultmgmtsp> fmadm acquit /SYS/MB/P0 faultmgmtsp> fmadm acquit /SYS/MB/P1 faultmgmtsp> exit
The faults should be gone after this.
Resolution: Update system to the Latest ILOM Firmware (SW 1.2 or later)

BIOS update in SW1.1.0 breaks ILOM PCI device slot inventory information |
With BIOS update in SW1.1.0 the slot inventory information changed as follows:
Before: PEM0 = 1, PEM1 = 2, REM = 3, FEM0 = 4, FEM1 = 5 After: PEM0 = 1, PEM1 = 2, FEM0 = 3, FEM1 = 4, REM = 5
This causes the /System/IO_Modules data to be missing or wrong:

X3-2B receiving false FMA SPX86-8003-QH faults with X1110A-Z installed |
When blade is booted with the X1110A-Z PEM card installed the PEM is faulted by FMA with one or both of these errors:
fault.io.intel.iio.pcie-downstream-devices fault.io.intel.iio.pcie-fatal
This issue has been root caused to incorrect BIOS settings for PEM1 in the BIOS code. |
Workaround: Clear the faults in ILOM using fault management shell
Note: The errors will return when the blade is rebooted or power cycled.
Resolution: Update system to the Latest ILOM Firmware (SW 1.2 or later)
