HCHalted bit is set - USB Host Controller has stopped running. Note: This will not affect the ability to boot the system from the disk or the network.
Occurrence
Note: Systems are only affected by this issue if they have a XVR300 graphics card present and a keyboard and mouse connected to the rear USB port where the DVD is also connected. If a system fails to boot, the presence of an XVR300 graphics card can be determined at the OBP prompt as follows: ok show-devs The list of devices output will contain a line similar to the following: /pc1@400/pci@0/pci@9/SUNW,XVR-300@0 Some mother boards and some revisions of system firmware appear to be unaffected by this issue.
