Symptoms

Commands such as disablesm, showdisk are failing on IB Switch.

[root@sw-ibb01 ~]# disablesm
-bash: /usr/local/sbin/disablesm: Input/output error
[root@sw-ibb01 ~]# service opensmd status
touch: cannot touch '/var/run/opensm.pid': Read-only file system
/etc/init.d/opensmd: line 249: read: read error: 0: Input/output error
opensm is stopped
[root@sw-ibb01 ~]#
[root@sw-ibb01 bin]# /usr/local/bin/showdisk
-bash: /usr/local/bin/showdisk: Input/output error
[root@sw-ibb01 bin]#

Cause

[root@sw-ibb01 etc]# showunhealthy
WARNING Flash disk has bad blocks
FAILURE - 1 sensors NOT OK
[root@sw-ibb01 etc]#

The root cause is Switch has a Bad Disk

Solution

Open SR with Oracle to get the IB Switch replaced.
