Servo did not reposition the drive within the allotted timeout.
 

Solution

If the T10000C tape drive is running the latest released drive microcode, 3.65.103 or higher, and the drive is failing on multiple pieces of media, this drive should be replaced. Servo integrity is lost. Please reference BUG 27951942.

Replaced the drive according (doc ID 2180418.1).
