Symptoms

ZFS-SA  Storage node in Fault State


Fault Fault critical Wed Nov 9 23:18:10 2016 Fault detected at time = Wed Nov 9 23:18:10 2016. The suspect component: /SYS/MB has fault.io.usb.device with probability=100. Refer to http://www.sun.com/msg/USB-8000-GT for details.

Solution

This is a benign fault for USB endpoint which occurs at Oracle Solaris boot.

Upgrade to Appliance Firmware Release version 2013.1.3 0 (or later)
