Symptoms

The server's Power On Self Test (POST) does not indicate any problem but a "probe-scsi-all" after a "reset-all" on OBP level shows:

Probing I/O buses
/pci@8,600000: Device 1 network
/pci@8,600000: Device 2 SUNW,qlc fp disk
/pci@8,700000: Device 1

ERROR: /pci@8,700000/<Unnamed>: Last Trap: Data Access Error


The server refuses to boot from any of the internal disk drives:

{7} ok boot disk0
FATAL: /pci@8,700000/<Unnamed>: OpenBoot initialization sequence prematurely terminated.


 

Cause

There is a problem with the IO Board.

Solution

Please open a Service Request (SR) at Oracle and attach the console log to the SR.
A Support Engineer will review the provided logs and discuss with you the visit of an Oracle Field Engineer in order to replace the server's IO Board.
