Symptoms

After following the instructions to apply <Patch 22546910> CREATE A PATCH WITH VIRTUAL IMAGE FOR X5-4 MACHINES, the following errors occur after rebooting the server:

qla2xxx [0000:a0.0]-d00d:7 firmware has been previously dumped (ffffc90018781000) -- ignoring request
qla2xxx [ 000:a0:0001]-00f5:7 unable to burst-read optrom segment (100/758fc400/80f94dc000)
qla2xxx [000:a0:00:0]-00f6:7 Reverting to slow read

Changes

Installed <Patch 22546910> on a new X5-4 OVM 3.3.3 server.

Cause

This behavior is caused by unpublished Bug 22341180 - Get qla2xxx error after applying OVM 3.3.3 and kernel-uek-3.8.13-98.5.2.el6uek.  This problem occurs on OVM 3.3.3 servers and is also linked to internal, unpublished <Bug 20745556> ISSUE WITH LACP BOND MODE WITH 1 GB NIC WITH DOM0_MAX_VCPUS=20.

Solution

To resolve these errors, do the following:

  1. Login to system as root user and make a backup copy of the /boot/grub/grub.conf file.
  2. Then use vi to edit the file and and modify the setting for "dom0_max_vcpus". Set this to be 8.  
  3. Save changes to the grub.conf file and reboot the server.
  4. Once the server is rebooted again, it will start up correctly.
