Cause
Port 2A on HBC card communication was dropped but no alternate path was available (Port B IP 10.145.219.5)
This makes me believe that there was/is a customer network issue involved with this.
Solution
VARY OFFLINE ACS
VARY ONLINE ACS
If problem still exist, ping each IP from HOST to check on customer's network environment.
