Cause

Server diagnostics do not pinpoint the failure of either MB, PCI_MEZZ or PCI card issue.
In this case replacing the motherboard, indicated to be the faulty component, did not resolve the issue. The solution was to replace the PCI_MEZZ instead.
 

Solution

Recommended steps to address the issue:

1. Replace PCI_MEZZ & clear fault
2. If issue persists, replace the motherboard
3. If issue still persists, remove all PCI cards
  3a. If no errors are seen with all PCI cards removed, reinstall one at a time and repeat test.
  3b. If the error is seen with all PCI cards removed, replace the pci card risers.

For each and every step above, ensure all faults are cleared (DocID 1004229.1) and set ILOM for the host to run MAX POST
