 show unhealthy command gives the following:

#showunhealthy
WARNING Flash disk has bad blocks   <<<<<<
FAILURE - 1 sensors NOT OK

env_test output also shows problems with internal disk:

Starting SSD test:
WARNING Flash disk has bad blocks   <<<<<
SSD test returned 1 faults
Environment test FAILED

And, ILOM snapshot shows:

Cause

 The internal hard disk of this switch has failed, or has bad blocks and needs to be replaced before full failure.

Solution

Internal hard disk is not a field replaceble unit.  The infiniband switch will have to be replaced.

So, the solution is to replace the infiniband switch
