Cause

Though in the panic string it would appear to be an IO issue, this panic was caused by an uncorrectable error in a DIMM or bank.

Solution

Open an SR with Oracle to determine whether this is truly an IO issue or DIMM.  In this case replace the failed bank, in this case Slot B: J2900 J2901 J3001 J3000
