Information in this document applies to any platform.

Symptoms

Issue was logged for the : fault.io.pciex /SYS/MB/PCIE0 & /SYS/MB/PCIE1

and the following warnings are logged in /var/adm/messages:

Nov 14 08:45:49 server1 qlc: [ID 630585 kern.info] NOTICE: Qlogic qlc(2): Link ONLINE
Nov 14 08:45:49 server1 fctl: [ID 517869 kern.warning] WARNING: fp(4)::GPN_ID for D_ID=10100 failed
Nov 14 08:45:49 server1 fctl: [ID 517869 kern.warning] WARNING: fp(4)::N_x Port with D_ID=10100, PWWN=21000024ff5483f0 disappeared from fabric
Nov 14 08:45:50 server1 fctl: [ID 517869 kern.warning] WARNING: fp(4)::N_x Port with D_ID=10100, PWWN=21000024ff5483f0 reappeared in fabric
Nov 14 08:46:25 server1 qlc: [ID 630585 kern.info] NOTICE: Qlogic qlc(2): Link OFFLINE

Cause

- The link OFFLINE was a result of "login fabric port failed" error.

Nov 14 08:46:25 server1 qlc: [ID 329025 kern.warning] WARNING: qlc(2): login fabric port failed D_ID=fffffch, rval=100h, mb1=300h 

- This error matches Bug 15816394, which according to the notes in the bug,  has been fixed in the latest qlc firmware.

Solution

- Install the latest qlc driver patch, currently patch <SunPatch:149176-08> . For latest qlc patches and firmware version see:

Note. You can check firmware version of Qlogic HBA using 'fcinfo hba-port'.  Below is an example:
