Symptoms

System won't boot. When trying to boot it would show one of these errors consistently:

ERROR: /pci@780: Improperly aligned address. function: b4

WARNING: /pci@780/pci@0: Standard Capability Config access failed

After performing a ok reset-all it may also fail to boot:

{0} ok boot disk0
Boot device: /pci@780/pci@0/pci@9/scsi@0/disk@0  File and args:
ERROR: boot-read fail

Can't locate boot device

{0} ok boot disk1
Boot device: /pci@780/pci@0/pci@9/scsi@0/disk@1  File and args:
ERROR: boot-read fail

Can't locate boot device

Sometimes a powercycle may clear the error and allow boot, but the issue will return at the next reboot.

Cause

 The issue is most likely a problem with either a disk or the controller.

Solution

If you've had history of issues with a specific disk (likely a boot disk), please reseat the disk.

You can also cycle the system completely by shutting it down, pulling the power cords, waiting for one minute, then plugging the power back.

If that doesn't resolve the issue completely, please open an SR with Oracle Support to investigate further.

 

NOTE: The customer for the SR that had this issue replaced the disk first and it didn't resolve the issue. Replacing the SB resolved the problem.
