Symptoms

Amber alert leds on Multiple disks

Cause

Current LSI firm ware version is too old (12.12.0-0048)

Solution

Download and install the latest LSI firmware (12.12.0-0178) from:
http://www.lsi.com/sep/Pages/oracle/sg_x_sas6-r-int-z.aspx

Reboot the server
