Symptoms

The kernel reports of a corrupt primary disk label in the messages file:

Apr  3 10:18:24 boli2 cmlb: [ID 107833 kern.warning] WARNING: /scsi_vhci/disk@g5000cca0227e4054 (sd4):
Apr  3 10:18:24 boli2   primary label corrupt; using backup

Format complains about the corrupt label:

Specify disk (enter its number)[0]: 1
selecting c0t5000CCA0227E4054d0
[disk formatted]
/dev/dsk/c0t5000CCA0227E4054d0s0 is part of active ZFS pool rpool. Please see zpool(1M).
Reading the primary EFI GPT label failed.  Using backup label.
Use the 'backup' command to restore the primary label.

Cause

The primary disk label was corrupted.

A possible reason for primary label corruption is from running the installboot command on SPARC servers containing Solaris 11.1.  The installboot command on a EFI label disk may corrupt the primary label of the disk, as follows:

Mar 15 10:46:18 server01 cmlb: [ID 107833 kern.warning] WARNING: /scsi_vhci/disk@g5000cca07d0b29c0 (sd6):
Mar 15 10:46:18 server01 primary label corrupt; using backup

On Solaris 11.1 & newer, use the bootadm command on an EFI (GPT) labeled disk.

For additional information, see doc:   How to Install a ZFS Boot Block (Doc ID 1020196.1)

Solution

Unfortunately, the man page for format doesn't state that it's backup function also attempts to repair the label which resolves the problem.

# man format
...
    backup       Search for backup labels.

format> backup
Restoring primary label.

This should resolve the problem, but if not then check document 1532506.1.
