Symptoms
Server will not power on or it will power on but show PSU voltage errors in the ILOM logs (-> show /SP/logs/event/list or sc> showlogs -v).

Cause
This is most likely a hardware issue. Could be a problem with the power distribution board.
Solution
Please perform the following:
1) Make sure your firmware is up to date to the latest. Firmware can fix many false positive problems and also sensor thresholds.
2) Make sure both power supplies are seated properly and that they are receiving ample power from the cords; make sure the power cables and outlet are working properly as intended
If the above doesn't resolve the issue, please open an SR with Oracle Support
The above issue was fixed by replacing the PDB. The customer also managed to open the box and verify that the Ribbon Cable is seated properly, but not all customers may be willing to check that themselves.
