Symptoms
An XVR-100 Card installed in PCI#2 no longer outputs to external display after updating from OBP 4.16.1 to 4.30.4.a with Patch 142707-01.
Server configuration and part numbers:
XVR-100 Frame Buffer card: 375-3181 pci slot 2
Ultra3 SCSI HBA: 375-3057 pci slot 4
motherboard 540-633
obp 4.16.1

1. When the server starts and probes devices, it detects the keyboard, then gives the message:
screen not found
2. With a laptop connected to ALOM:
When the server boots, keyboard does nothing. After the obp settings are applied, the keyboard connected to the server inputs to the laptop connected to the ALOM.
3. From the OBP, after motherboard replacement:

There may be some part number dependencies on this issue happening. Exhaustive testing with different parts and revisions was not performed due to parts limitations.

Changes
The issue was triggered by upgrading the OBP from 4.16.1 to 4.30.4.a.
The following hardware was replaced with no change in the XVR-100's behavior:
-motherboard
-XVR-100 card -both the RoHS and non-RoHS versions
-Ultra3 scsi card
-Firmware was upgraded and downgraded - the issue was not resolved by downgrading the firmware back to 4.16.1
Cause
As PCI slots 2 and 4 are on the same bus, there seems to be a conflict with the Ultra3 and the XVR-100 when installed in these slots. There may be some part number dependencies on this issue happening.
The XVR-100 was visible again and functioned normally with the Ultra3 card moved from pci slot 4 to pci slot 5. There was no mix of hardware or firmware configurations that worked to restore operation to the XVR-100 card with the ultra3 scsi hba installed in pci slot 4.
Solution
This issue was resolved with the workaround - moving the Ultra3 card from pci4 to pci5.
SR 3-6173440864 was a similar issue - The XVR-100 stopped outputting to the monitor after updating the obp from 4.22.33 to 4.30.4.a. The other SR was resolved by downgrading the firmware back to 4.22.33, so it may help to downgrade firmware before moving or replacing hardware.
