The server has been rebooted and the upgrade process was retried a few more times. Yet, the problem persists.

The KMA with OKM 2.3.1 was installed over a year ago and this is the first time that the software is being upgraded.

Changes

Upgrading OKM to the latest release

Cause

Some KMA's manufactured a year or so back have disks that were partitioned incorrectly.
The problem comes up during the upgrade process.

Solution

Replace the KMA disk.
The KMA disk may be loaded with OKM 2.3.1.  If this is the case, run the
software upgrade process to update the OKM software to the desired release level.
