Cause

Cleaning of the drive fails and as a result all the backups to the drive fail with various errors like I/O error, read write error etc.

Solution

Clean the drive using a new or a known good cleaning tape. Perform a test backup with new or known good data tapes and monitor the drive for a couple of days.
