Rebuilding /boot/initrd-2.6.32-400.21.1.el5uekkdump.img
no record for 'dm-0' in database

Unpublished Bug 16789676 : MKDUMPRD FROM KEXEC-TOOLS-1.102PRE-161.EL5.X86_64 FAILS WITH EXT3.KO SEARCH
 

 

Solution

Download a different kexec:

kexec-tools-2.0.3-3.0.4.el5.x86_64.rpm  (available at https://oss.oracle.com/el5/SRPMS-updates/)

You have to remove the current kexec tools using

rpm -e --nodeps kexec-tools


Then install new one with

rpm -i <path_to_kexec-tools.rpm>
