Symptoms

Receiving CAM/FMA alerts for USB-8000-CL

 100%  fault.io.usb.not_acc

There are no functional or operation impacts to the VSM6 or VSM7 systems with this error.  The VSM appliance does not use USB ports and this is a benign informational error in a VSM system.

Cause

This is a known message, especially on older microcode. 

Solution

There are no functional or operation impacts to the VSM systems with this error, this is a benign error in a VSM6 system.

Clear all USB errors and install the latest microcode.

For VSM6 systems with code levels lower than 6.0.6.05.000, there is a manual workaround to disable the alerts, please contact VSM support.

Pre 6.0.6.05.000 manual workaround is as follows, to be performed only by Trained Oracle VSM6 support engineer:

1) From ILOM, clear and acquit the USB fault (refer to the ILOM Maintenance Guide)
2) Log onto each VSM6 node, and do the following to to unconfigure the the keyboard and mouse ports:
    # su root  (use default password of changeme!)
    # cfgadm -c unconfigure usb2/2.4

The above steps will disable the mouse and keyboard ports without affecting other USB ports on the server.
The above process is not persistent across re-boots and would need to be re-entered after a server reboot.
