Symptoms

Drive failing to read multiple tapes.

ASC: 0x11 (unrecovered read error), ASCQ: 0x0, FRU: 0x0

Cause

Possibly the read/write head has worn out. 

Multiple known good tapes fail to be read in the drive.

A Read operation failed. This is probably due to bad media but may be hardware related.

Solution

Check Error Warn Info to see if there are any media being reported as bad. Or the same tape failing in multiple drives.

Perform HP LTT (Library Tape Tool) test on the drive using a known good tape.  Check if the same tapes can be read in other drives.

Replace the drive if the test fails with a known good tape cartridge.
