This indicates that multiple tapes have failed to load in to the drive.  Also the Source Slots are variable.

The possible cause is with the drive loading mechanism.

Inspect the tapes with reported errors.

Check if the drive loading mechanism is intact.

Try loading a known good and compatible tape in the drive.

If the load fails, replace the drive.

Example of Action Plan:

Drive: 15 ie: 0,5,1,9 is giving load errors on multiple tapes from various locations.
It appears that the drive loading mechanism may have failed.
Or there could be possible obstructions as the tape tries to load to the drive.
Check if there is any obstructions as the tape tries to move in the drive through Robot hand.
Check if the Drive Bezel is intact.
Load a couple of known good and inspected tapes in the drive and if they fail to load, please replace the drive.
If the drive bezel or any other component seems to be defective, please order the same through dispatch team.

In some cases, one may see tapes failing to load in multiple drives.  In that case it may be a media problem.
