Cause

Possible issues:
1. Media damaged.
2. Drive damaged.
 
Physically damaged media or drive can cause problem in loading.

Solution

From the SL24/48 Error codes manual) .. An 80 F4 .. code dictates the following action.

1. Update library firmware to the latest revision. Firmware enhancements have been added which may prevent the condition that causes this error
2. Check that the tape cartridge is supported and has not exceeded its usage life. Inspect it for damage.
3. Retry the operation.
4. Clean the drive.
5. If the issue continues, remove the drive and inspect the opening for any obstructions

The '91' dictates...
'91 Elevator movement to home position failed'.

In this case you need to pull the drive straight away and inspect it since cleaning etc will probably do nothing (always worth a try though).

1. Try loading a couple of known good tapes in the drive.
2. If media works fine in another drive and not in the drive in question then replace the drive.
3. If media does not work in any drive then replace the media.

If suspected drive issue follow below steps before drive replacement.

1. Drive power-cycle and cleaning can be done as basic troubleshooting.
2. Check the current firmware level and upgrade drive firmware to latest.

The above 2 steps could resolve the issue without replacing the drive. If problem remains after following above steps, it could be a mechanical problem in the drive, consider replacement.
