Symptoms

During an R5.x to R5.x software update, it's possible for the software update to fail due to a "FAILED_TO_DRAIN_OPERATION_POOL" event.  This event can be seen by accessing the System Alerts page and double clicking on "Software Update Failed" alert.

Cause

When the FAILED TO DRAIN OPERATION POOL event occurs, it means that the operation pool is hung up.  This can happen if a running process has not completed, for example if there are log bundles queued up for transfer that have not completed.

To check for processes that are currently running.  In the GUI click on the "Tasks" button located bottom right, check to see if there are any active processes running that have not completed.

NOTE: There are no options for a user to forceably stop a running process in the GUI.

Solution

To resolve the issue:

In the "Update Software" screen under "Software Update Options" check mark "Ignore current requests: Ignore ongoing requests, force pilot shutdown and continue the software update process" and "Override failed software update: Override a previously failed software update":

Software Update Options

Proceed with the upgrade again, the upgrade will still be non-disruptive.

NOTE: You do NOT need to re-stage the software package.

The alternative solution is to force a pilot failover by running the following pcli command which must be done by Oracle Support:

pcli sub -H <Axiom/Pilot IP> -u pillar -p a1s2d3f$ ForcePilotFailover

Then log back into the GUI and re-run the upgrade.
