Symptoms
ASR Problem description shows:
The Oracle Proactive Support Center has reported that serial number 516000200100 has exceeded the daily error threshold for event 3337/3357 on the following volsers.
Exceeding this threshold may indicate a failing cartridge or drive in the SL8500/Sl300 library. Please follow the RDC process to diagnose whether this is a drive failure or a
suspected cartridge and dispatch to the field if necessary.
A drive reports "load failed" errors followed by "rewindUnload Successful with mediaError" warnings.
For example:

Cause
Any of these conditions would cause these load errors:
1. The tape may not have been properly rewound from last mount and had a slack.
2. The tape failed to unslack during a load.
3. The cartridge leader may be pulled inside the cartridge housing.

While track following, we detected an incorrect Band ID.
This means the head is in the incorrect Band
Solution
Ask the customer to dismount the tape from the library control software or from the backup application.
If the dismount fails, have the customer try running the dismount command with the force-unload option.
If the tape does not unload successfully, it is most likely stuck and would need a field engineer to remove it;
Eject the cartridge and check for leader issues
The tape probably has the leader pulled inside the cartridge. Have the customer eject the cartridge, pull open the cartridge window and verify whether the cartridge loop is seated in the correct position.
For T10000A and T10000B drives, there are several possibilities for this problem:
1) Tape leader may be pulled into the cartridge. Open the door of the cartridge and verify that the leader is seated in the tabs on the side properly.
2) The tape may have too much slack inside the cartridge. obtain a clutch tool and manually take the slack out of the cartridge.
3) The tape may be broken inside of the cartridge. Obtain a clutch tool and manually take the slack out of the cartridge.
If the slack cannot be removed then the tape is probably broken. Contact the data recovery lab for further instruction on tapes like this.
