Cause

A host prevented the tape from being unloaded.
This situation can happen for example when a job is aborted leaving the drive with  the prevent medium removal bit set.

NOTE:
Prevent Allow Medium Removal, (PAMR), is a common SCSI command across all our tape drives. You will find this PAMR cdb listed in all the interface reference manuals for LTO, 9840, T0K.

Solution

A host prevented the tape from being unloaded. This is what the Tape Alert tells:

PREVENT/ALLOW MEDIUM REMOVAL 1Eh
PREVENT/ALLOW MEDIUM REMOVAL tells the drive to enable or disable the removal of the cartridge.
When cartridge removal is prevented, the front panel eject button is completely disabled (though a ‘forced eject’ will still work). An Unload command issued by a host will result in CHECK CONDITION.
The sense key will be ILLEGAL REQUEST or MEDIUM REMOVAL PREVENTED.
When cartridge removal is enabled, pressing the front panel eject button or sending an Unload command from a host will cause the media to be unloaded and ejected.
Removal prevention is on a per-host basis. All the hosts have to allow media removal for any of them to do so.

One way to clear this is to power cycle the drive which is somewhat agressive against a loaded drive.

Regarding HP LTO , (although a ‘forced eject’ will still work), you can use HP LTT software to force a graceful eject as shown below:

Manually eject the tape using the "Eject" button on the front of your LTO-3.
If the manual ejection fails, launch HP's Library and Tape Tools utility -- (install the program if you haven't already).
- Click on the program's "By Connection" tab.
- Select your LTO-3 from the list of options under the tab.
- Click the "Utility" heading at the top of the program.
- Select the "Run Utility" option under the "Utility" heading.
- Select the "Force Tape Eject utility" from the "Utility" menu box, when the "HP L&TT - Run Utilities" menu appears.
- Click on the "Run Utility" button to force the tape out of the drive.

Note:
This force eject utility is unique to HP.
