Cause

 Unplugged internal SCSI cable.

Solution

Resolution

The motherboard was replaced, this did NOT resolve the problem.
SOLUTION:
The customer had recently purchased an external scsi card for
this sunblade.
There were no installation instructions with this card.
During installation, the customer removed the internal scsi cable
(ribbon cable) from the motherboard and plugged it into the
connector in the external scsi card.
Now the system could no longer find the bootdisk.
Once the internal scsi cable was plugged back into the
motherboard, the system booted correctly.
