Affected Parts: (FRU/CRU Part Number / Description)

7070758 - A254 7100W 3-Phase AC Input Power Supply
7074835 - A254 7100W 3-Phase AC Input Power Supply
7074836 - A254 7100W 3-Phase AC Input Power Supply

Symptoms

PSU faults with ‘UV_FAULT’ (Under Voltage) failure signature that indicates failure due to possible shorting/arcing of internal PSU components due to contamination.

Impact

Failed PSU(s) do not cause Domain(s) or Platform to shutdown. Twelve PSUs provide 1+1 (2N) redundancy, allowing for a single Grid Failure (6 PSU) as long as the FANS on the PSU are running.

Changes

Shorting/arcing of internal PSU components due to contamination, typically from dirty environments. Only PSU part numbers listed above, and with a datecode earlier than 1430, are affected by this issue.

Cause

Root cause attributed to contamination from dirty environment. New part number 7305956 for the A254 has been released that implements conformal coating and MOSFET lead protection as a preventative measure to address this failure mode.

Solution

Target Completion Date: 23-Sep-2017
Hot Swappable? Yes (if only one PSU replaced at a time)
Estimated FE Time: 5 hours onsite per system (plus travel)

Workaround

No workaround available - see Resolution section.

Resolution

Proactively replace all affected A254 Power Supplies (PSUs) with new A254 PSU (p/n 7305956) using the instructions in the below "how to" section. Although there are twelve (12) PSUs per system, they may not all need to be replaced.  A254A PSU p/n 7321272 may be used when the 7305956 is not available.

First review the FCO 348 Deployment Process document before proceeding further, as the Domain will contact the customer upon SR creation to validate the number of PSU required.

The Corrective Action Plan may be reviewed here; Mx-32 - How to implement PSU Replacement for FCO A0348 (Doc ID 2055851.1).

Note: All DOA replacement parts should be CPAS'd for analysis. If needed please contact TSC/Domain for details on the CPAS process.

Comments

An impacted End User List is available in APEX for identifying and tracking remediation activities. Please contact your Regional Deployment Manager for End User information.

Parts Return Instructions

Each replaced PSU should be returned with its own Parts Return Label.

FE Onsite Parts orders

Please ensure that any PSUs that were replaced (but did not experience a failure) be marked as BAD/SUSPECT with 348 in the FCO# field on the FE Onsite Parts Return Label.

For failed PSUs mark as BAD/SUSPECT and leave the FCO# field blank.

Partner/Customer Parts Only orders

Please ensure that any PSUs that were replaced (but did not experience a failure) be marked as BAD and write FCO 348 in the top right hand corner of the Parts Only Return Label.

For failed PSUs mark as BAD but do not write the FCO number anywhere on the label.

Note: All DOA replacement parts should be CPAS'd for analysis. If needed please contact TSC/Domain for details on the CPAS process.

Identification of Affected Parts (how to):

- How to determine the Part Numbers of affected PSUs in a system?

PSU Part Numbers that Does Not Require FCO 348 replacement

7321272 Does Not require replacement - has Conformal Coating
7305956 Does Not require replacement - has Conformal Coating
*7074836 Does Not require replacement for Serial Numbers dated Week 30 2014 and higher

PSU Part Numbers that Requires FCO 348 replacement

*7074836 Requires replacement for Serial Numbers dated Week 29 2014 and below
7074835 Requires replacement
7070758 Requires replacement

How to determine Week Number from a given Serial Number for PSU Part 7074836?

1. Obtain the Part and Fru/Board Serial Number from either of the two methods provided below
2. If the Part Number is 7074836, obtain the Serial Number
3. In this example, the Serial Number is 1430C56650 (omit the first portion which says 465824T)

Board Serial : 465824T+1430C56650

4. The first 2 digits of the serial number tells us Year as 2014 and the next 2 digits tells the Week Code as 30

5. Only for Part 7074836, if the SN is dated Week 30 2014 and higher - DO NOT replace

6. Only for Part 7074836, if the SN is dated Week 29 2014 and below - Replace it

 - How to check for Part and Serial Numbers?

Part Numbers that need replacement are: 7070758, 7074835 and *7074836.
Note: Part Number 7074836 with date code 1430 and above
Does Not Require Replacement.

Two Methods are provided to Identify the Affected Part Numbers and Date Code 1430 and above.
