Symptoms

If more than one card fails, the VSM5 system operation may be severely degraded or non-functional.

Impact

VCF4 cards in the VSM5 may fail to initialize correctly on power up, necessitating replacement of the card.

Specific VCF4 cards shipped in the VSM5 platform as identified in the Customer List are impacted. The following VCF4 card serial numbers and serial number ranges (not containing GSAP 5627 (or later) label) are impacted:

If GSAP 5627 label is found on the card it has been repaired and is not impacted.

Design defect in the Micron DDR memory. The design flaw was corrected by the vendor in chip as of date-code 49-2011 (and later). Regional Stocking Locations (RSLs) were purged beginning on September 1, 2011 via GSAP 5627.A.

Solution

Target Completion Date: May 28, 2013

Note: As of the above date all impacted cards have either been replaced, customer has refused remediation, or system/cards were not found, so remediation of this FCO has switched from Proactive to Reactive (or Fix-on-Fail only).

Workaround

The only work-around is to avoid power cycling the VSM before affected cards have been replaced.

Resolution

Proactively replace suspect VCF4 cards per customer list and if card serial numbers are in the affected ranges listed above. Refer to the VSM service manual for detailed procedures. Affected End-User Customer Lists are being managed by, and available from, the Regional FCO Deployment Managers listed below.

Do not proactively engage any customer to begin remediation activities without first obtaining agreement from your Regional FCO Deployment Manager, who is micro-managing this deployment due to limited available material.

Defective Material Return Instructions: check Bad/Suspect box on Return Label and write 316 to the right of FCO# field.

Identification of Affected Parts (how to)

VSM5 systems which may have one or more VCF4 cards with chips with the suspect date codes are listed in the customer list.

Follow the below steps in order to identify the VCF4 cards in a system.

Display VSM5 VCF4 Serial Number

1. Connect to the VSM5 using DOP (Detached Operator Panel)
2. Log on to the CSE Main Menu
3. Select Configuration
4. Select FRU Status
5. The VCF cards will be listed on the first page displayed. There will be 4, 6, or 8 VCF cards displayed
     depending on the machine configuration.
                a. Check the Card Type column for VCF4 only.
                b. For each VCF4 listed, document the associated Location, Part Number, and Serial Number.

Note that the FRU ID only lists six digits, so only those are displayed on the panel and in the machine FRU file. Compare VCF4 card serial numbers from the above steps to those shown in the Contributing Factors section above.

Important! If the card contains GSAP 5627 label it has been repair and is not impacted by this issue.

Hardware Remediation and Material Availability Details

Seedstock is limited and therefore remediation is divided in two phases. The first phase will be to proactively remediate systems identified in the Phase 1 tab of the Customer List, and then remediate all other impacted, contracted systems identified in the Phase 2 tab of the Customer List.

Comments

Ensure you do not remove the compact optical transceivers or SFPs (Small Form-Factor Pluggable) from the defective VCF4 cards. When these cards are returned missing these components it drastically extends the repair cycle of the VCF card.
